Patient Forms

We have decided to go paperless and make all of our forms electronic so our patients can fill them out before coming to our office. Any form you wish to fill out will be sent directly to one of our staff and the information will be added to your file. If you are a new patient, we will create a file for you.

1. New Patient Form- Fill out this form if you are a new patient with us. We will create a file for you and contact you to confirm your appointment.

2. Sleep Apnea- Fill out this form to find out if you may be at risk for obstructive sleep apnea.

3. TMD Questionnaire- Fill out this form if you are having jaw problems or experience and pain when chewing or moving your jaw.

4. TMD Pain Analogue Scale- Similar to the previous form but examines our patients severity of pain if they have any problems with their jaw.
